Every Man for Himself

In the flashback, Sawyer continues to con people by gaining their trust and then backstabbing them. In this instance, Sawyer is in prison and juggling the loyalties of a wealthy inmate and a corrupt warden. Back at the Others' camp, Sawyer seizes on the chaos that erupts after the Other 'Colleen' is shot by trying to electrocute Ben. Since the Others are able to observe Sawyer and Kate in their cages, Ben knew about Sawyer's plan and shut off Sawyer's electrical conduit. As punishment, Ben beats Sawyer with a baton, knocking him unconscious. When Sawyer comes to, Ben tells him that they have inserted a pacemaker into Sawyer's chest that will kill him if his heart rate goes above 125bpm.

The wounded Other Colleen is brought back to the Others' camp, and Juliette tries to get Jack to save her life by operating on her. While Jack is in the operating room, he sees some x-rays on the wall depicting a large tumor - Jack is intrigued. Colleen dies on the operating table despite Jack's best efforts, and an outraged Pickett takes his anger out on Sawyer, beating him to within an inch of his life. He is saved when Kate steps in tells Pickett she loves Sawyer.

Back in the operating room, Jack is handcuffed to the table containing Colleen's body. When Juliette comes back in to fetch Jack, he confronts her about the X-Rays he saw earlier, asking her who they belong to.

Ben takes Sawyer on a hike and shows him that, in fact, they are on a separate island a couple of miles from the mainland, where the rest of the survivors are. He explains that even if Sawyer did manage to escape from his cage, there would be nowhere for them to go.
