Not in Portland

The episode opens with Juliet sitting on the beach, crying. She leaves the beach and walks down a corridor - passing known "Other" Ethan Rom - and into a room, where she prepares a shot for a sleeping woman. It is revealed that this is her sister, Rachel. Juliet pulls the curtains open - it turns out they are in Miami, not on the island.

Kate and Sawyer are fleeing captivity, while Juliet sends a group of Others led by Pickett to capture Kate and Sawyer - 'and kill them if necessary.' She also tells Jack that Kate and Sawyer couldn't even escape if they wanted to - they are on a separate island. This infuriates Jack, who tells Tom that Juliet had asked him to kill Ben.

In the second part of the flashback, Juliet is caught in the medical lab she works in by her boss Edmund - who is also her ex-husband - stealing vials of the drug that she was seen giving to her sister in the opening. Edmund threatens Juliet - saying that if she doesn't let him in on whatever she's doing scientifically, there will be repercussions.

In the jungle, Kate and Sawyer are almost killed by the Others when they are suddenly saved by Alex, who leads them to a hiding place in the jungle. In the surgery room, Ben wakes up in the middle of his surgery - he has heard about Juliet's betrayal and Jack's plan to let him die if his friends are not saved. Ben tells Juliet that they need to let Sawyer and Kate go.

In the third part of the flashback, Juliet has a job interview with a mysterious company called Mittelos Bioscience. They have been impressed with her research into fertility and want her to come work for them. She tells them her ex-husband would never let her go, the only way she could work for them would be if he were hit by a bus.

In the jungle, Alex tells Sawyer and Kate they can leave once they help her boyfriend Karl escape. The gang goes into a strange room where Karl is being held prisoner and forced to watch strange images on a movie screen.

In the flashback, Juliet tells her sister about the job interview, and Rachel reveals she is pregnant - which was thought to be impossible. Juliet is amazed - she hasn't failed in her research. Elsewhere, Edmund is suddenly hit by a bus and killed.

Kate, Sawyer, Alex, and Karl make it to the shore and a waiting canoe and prepare to leave. Suddenly, Pickett and the others show up. Just before Pickett is about to shoot Sawyer, Juliet appears and shoots Pickett, killing him. Juliet tells Kate and Sawyer they can leave, but Alex and Karl have to stay. Kate and Sawyer take off in the canoe, though soon after Kate wants to turn back for Jack.

In the last part of the flashback, Juliet goes to the morgue to identify her ex's body. While she is there, the reps from Mittelos show up and tell her again that they want her to work for them, urgently. Juliet is in shock - thinking Mittelos might be responsible for her ex's death - and she asks if she can bring her sister with her if she goes to work for them. They say it might not work, since - contrary to what they told her earlier, their offices are actually 'not in Portland' after all.
