The first segment of the flashback sees John Locke, pre-paralysis, being cut off from government benefits. He returns home to his small apartment, depressed and defeated, when he gets a knock on his door. It's a young guy named Peter, who explains that his mother is set to marry a man named "Adam Seward." Peter says he's been suspicious of Seward and decided to look into the man's past - Locke's name came up when he found that Locke had donated a kidney to Seward some time ago. Locke feigns ignorance and sends Peter on his way.
On the island, Locke, Sayid, and Kate are still confused by Jack's strange behaviour - he seems to be friendly with the Others. Locke suggests they wait until sundown before they try to approach Jack. When darkness falls, Kate slips into Jack's room and finds him playing a mournful song on the piano. He is shocked to see her, and yells at her for disobeying his order to not come back for him. Meanwhile, Locke goes into Ben Linus' room and points a gun at him. When Ben's daughter Alex comes into the room, Locke holds her hostage.
Ben asks Locke what he wants, and Locke tells him that he wants the Others' submarine. When more Others show up, Locke ducks into a closet with Alex. Ben tells the Others to bring him "The Man from Tallahassee." Locke has Ben make Alex go retrieve his backpack, which the Others' confiscated from Sayid.
In the next segment of the flashback, Locke tracks down "Adam Seward" - a.k.a. his biological father - and confronts the man who conned him out of his kidney. He tells Seward that he will reveal everything if Seward doesn't call off the wedding, which Locke is convinced is an elaborate scam for the wealthy woman's money.
At the Others' camp, Kate and Sayid are captured. Kate is brought to a room in handcuffs, and Jack comes in to see her. Jack reveals the truth - he has struck a deal with the Others and is set to return home the next day. He tells Kate he will come back for her. Juliet comes to the room to fetch Jack - they are both leaving in an hour.
In the flashback, Locke is visited by some police officers. Peter, the son of the man that Locke's father was set to marry, has been murdered. Locke is in shock - he is sure that his father had a hand in the young man's death.
In Ben's room, Ben plays mind games with Locke as the two sit and wait for Locke's pack to be brought back. Ben seems to know that Locke wants to blow up the submarine - he theorizes that Locke doesn't WANT to go home at all, where he would be paralyzed. Ever since the island healed him, he has been a changed man. Ben asks Locke about his paralysis and Locke gets frustrated. Ben also tells Locke about something strange on the island - a "very large box" that can manifest anything one wishes for.
In the flashback, Locke confronts his father about Peter's murder. His dad denies any involvement, but when Locke turns his back on his father, the con-man charges Locke and pushes him out an 8-story window. Locke crashes to the ground, mangled and maimed.
On the island, Locke receives his pack, which is filled with explosives. He leaves Ben's room with Alex and heads straight for the submarine. At the same time, Jack is being led with Juliet down to the sub to leave the island. Right as they arrive, prepared to leave, Locke detonates his C4 and the sub is destroyed. Ben's mind games paid off - he kept his word to Jack that he would let him go, but now that is impossible. The Others capture Locke and put him in a dark room.
In the final part of the flashback, Locke sits in a hospital room. An orderly comes in and says it's amazing that Locke lived considered how far he fell. But he broke his back - Locke is now paralyzed.
On the island, Ben visits Locke in captivity. He tells Locke that he wants to show him what came out of the mysterious box that he mentioned before. He leads Locke down a hallway and opens the door - to find Locke's father, bound and gagged, looking at Locke in utter shock and terror.
