Flashback intro: Kate is standing by the side of the road in Iowa. A tow truck takes her to a service station, where she sees a woman trying to sell fake jewelry, who turns out to be Cassidy - the woman that Sawyer bilked out of her life savings years earlier before leaving her, pregnant with his child. The owners of the service station threaten to call the police on Cassidy, when Kate steps in and helps her out. Cassidy recognizes that Kate helped her because she didn't want police attention.
At the Others' camp, Kate is being held in some kind of game room. She plans to sneak up on and attack Juliet when Juliet comes in to give her lunch, but Juliet is prepared and actually manages to throw Kate to the ground, hurting her. Locke comes in and tells Kate that he is "leaving" with the Others, but he doesn't specify where to. Kate is confused - Locke tells her that he tried to convince the Others to take her with them, but that they told him that she and Jack would have to be left behind.
At the beach, Hurley tells Sawyer that they are going to have a vote to get him moved down the beach away from the rest of the group, "banishing" him. Hurley says he can sway the vote if he starts being nice to people.
In the flashback, Kate has a drink with Cassidy and tells her she has come to Iowa to try to talk to her mother. She tells Cassidy the truth about killing her stepfather, and Cassidy says she will help Kate. Cassidy explains she was hurt herself by a man and she understands Kate's pain. She tells Kate that she was conned out of her life savings.
On the island, Kate is still sitting in the game room when a gas canister comes through the window. Kate looks out and sees The Others fleeing en masse, with gas masks on and suitcases. She passes out. Kate wakes up later in the jungle and finds that she is handcuffed to Juliet.
On the beach, Sawyer starts being nice to everyone, buttering up Claire and hunting a boar with Desmond so they can have a feast.
In the flashback, Cassidy goes to Kate's mother's house dressed as Kate. FBI agents appear instantly and throw Cassidy on the ground. They are shocked when they realize it isn't Kate - Cassidy tells them she is just a bible saleswoman.
On the island, Juliet claims to have no idea what has happened, but Kate is suspicious. Juliet says they have left her behind as well. They begin to make their way back to the Others' camp, and Juliet tells Kate that the real reason that Jack told Kate not to come back for him was that he had seen Kate and Sawyer having sex in the cage - Juliet says that Kate broke Jack's heart. Kate looks devastated.
Suddenly, the Black Smoke appears and comes face to face with the women - flashes of light appear before Juliet's eyes, as though it's taking pictures of her face. Juliet acts as though she's never seen the Smoke Monster before.
In the flashback, Cassidy goes to the diner where Kate's mother works and "accidentally" spills food on Kate's mother's clothes, so Kate's mother makes her way into the toilet, where Kate is hiding in a stall. Kate appears and asks her mother why she turned her in after the death of Kate's stepfather. Kate's mother says that he may have been a bad man, but she still loved him. She tells Kate that if she ever sees her again, she'll have her arrested.
On the island, Sawyer realizes that Hurley made up the story of the vote and the banishment just so he could get Sawyer to realize the benefits of being nice. Sawyer realizes that he has been conned.
Kate and Juliet are chased by the Black Smoke. They approach the sonic fence, and suddenly Juliet produces the key for the handcuffs. Kate is shocked that Juliet had the key with her all along. Juliet says she just "didn't want to get left behind again." They make their way to the Others camp and find Sayid. Kate locates Jack, passed out in a trashed house, and explains that she saw all of the Others flee.
At the end of the flashback, Kate and Cassidy sit in a car. Kate asks Cassidy to give her the name of the guy who conned her out of her life savings. Cassidy admits that she is pregnant with the man's baby. Cassidy asks Kate if she will ever forgive her mother for trying to sell her out to the police, and Kate indicates that she never will. They part ways.
At the Others camp, Jack says they should get going back to the beach to join the survivors. Sayid doesn't like the idea of Juliet coming with them, but Jack is adamant. They leave for the beach.
